Hiperfrecuentadores versus Normofrecuentadores en atención primaria de salud: factores sociales asociados, estilos de personalidad y psicopatología

Abstract

El objetivo de este trabajo consiste en analizar si existen características sociales y psicológicas comunes a los hiperfrecuentadores, que los diferencien de los normofrecuentadores, y que puedan explicar la necesidad de acudir repetidamente a las consultas. Con dicha finalidad, se ha llevado a cabo un estudio descriptivo, transversal y retrospectivo, en el Servicio de Atención Primaria (SAP) Esquerra de Barcelona. Entre el 1 de septiembre de 2007 y el 31 de agosto de 2008, hemos evaluado mediante entrevistas personales a 129 pacientes clasificados como hiperfrecuentadores (visitas CP y/o URG >= 18 (percentil 95 de la distribución de visitas CP y/o URG de los usuarios del SAP) y a 109 pacientes clasificados como normofrecuentadores. Ambos grupos se aparearon por edad y sexo. Se administraron los siguientes instrumentos: cuestionario de datos sociodemográficos, el cuestionario APGAR familiar, la escala de apoyo social funcional de Duke, las escalas de ansiedad y depresión de Goldberg, y el Inventario Clínico Multiaxial de Millon (MCMI). Los resultados obtenidos de la regresión logística parecen indicar que los usuarios que presentan ansiedad subclínica tienen más del doble de riesgo de ser hiperfrecuentadores. En relación al MCMI, puntuaciones elevadas en personalidad Dependiente aumentan un 10% el riesgo de hiperfrecuentación y puntuaciones bajas en personalidad Compulsiva reducen un 2% dicho riesgo, mientras que puntuaciones elevadas en el Trastorno de ansiedad o en Depresión mayor aumentan un 10% el riesgo asociado a una mayor probabilidad de manifestar hiperfrecuentación. Los resultados de este estudio nos permiten conocer mejor a dicho colectivo y sugerir planes de intervención psicosocial para paliar el sufrimiento de estas personas y permitirles una plena integración social.


The aim of this study was to analyze if there are any social and psychological features common to frequent attenders which may differentiate them from no frequent attenders, and if these characteristics may explain the need for repeatedly attending consultations. For this purpose we conducted a descriptive, cross sectional and retrospective study on a Primary Health Care Service "SAP Esquerra" in Barcelona. From September 1st, 2007 to August 31st, 2008 we conducted 129 personal interviews to participants classed as frequent attenders [previous appointments and/or out-of-hours visits>=18 (95th percentile of distribution of PA and/or OV of PHCS users)] and to 109 participants classed as no frequent attenders. Both groups were matched by age and sex. The following instruments were administered: Sociodemographic data index, the Family APGAR index, the DUKE-UNC Functional Social Support Questionnaire, the Anxiety-depression Goldberg Scale, and the Millon Clinical Multiaxial Inventory (MCMI). The results obtained from the logistic regression seem to indicate that those who present subclinical anxiety are more than twice at risk of being frequent attenders. Regarding to the results from the MCMI, high scores on Dependent personality increase hyperfrequentness 10%, whereas low scores on Compulsive personality reduce the above mentioned risk 2%; and high scores on Anxiety disorder or on Major depression may increase 10% the risk associated to frequent attendance. The results of this study allow us to learn more about this group and suggest plans of psychosocial intervention to relieve their suffering allowing them a full social integration.
