Abstract

Introducció La prevalença d’hipogonadisme hipogonadotrop (HH) en pacients amb diabetis mellitus tipus 2 oscil.la entorn un 17-30%. En aquests, el tractament amb testosterona ha demostrat millorar la sensibilitat a la insulina, el control metabòlic i els factors de risc cardiovascular. Pocs estudis han examinat el seu paper en pacients amb diabetis mellitus tipus 1 (DM1). Objectius Determinar en una cohort de pacients amb DM1 la prevalença d’HH i els factors relacionats, així com avaluar l’associació entre la prevalença de cintura hipertrigliceridèmica, els factors de risc cardiovasculars i la presència d’HH. Avaluar l’eficàcia del tractament substitutiu amb undecanoat de testosterona (UT) en pacients amb DM1 i HH, sobre la sensibilitat a la insulina mesurada segons la taxa estimada de disposisió de glucosa (eGDR), el control glucèmic, els paràmetres antropomètrics, el perfil lipídic i la pressió arterial (PA), així com els canvis en l’Índex Internacional de Funció Erèctil. Mètodes Estudi transversal multicèntric en el qual s’inclouen individus majors de 18 anys amb diabetis mellitus autoimmue. Per avaluar la cintura hipertrigliceridèmica s’ha creat la variable “concentració de triglicèrids (mg/dL) + circumferència abdominal (cm)” estratificada en quartils. Disseny d’un estudi aleatoritzat, doble cec, controlat amb placebo i multicèntric que inclou pacients amb DM autoimmune i nivells de testosterona total <10 nmol/L i/o testosterona lliure calculada mitjançant la fórmula de Vermeulen < 225 pmol/L, amb LH/FSH normals o baixes en dues mostres sanguínies. Els pacients s’han randomitzat a rebre tractament amb UT 1000 mg intramuscular o placebo en 3 dosis (basal, 6 setmanes i 16 setmanes). Resultats Es van icloure 181 pacients. La prevalença d’HH va ser del 8,3 % (95% intèrval de confiança [IC]: 4.3-12.3%). L’edat (odds ratio [OR] 1.066 [95% IC: 1.002-1.134]), la circumferència abdominal (OR 1.112 [95% IC: 1.028-1.203]), i els requeriments de insulina ([UI/Kg] x 10 [OR 1.486 {95% IC: 1.052-2.098}]) van ser les variables associades de forma independent amb la presència d’HH. El model que millor va prediure l’HH va generar la següent fórmula: HH-score = (1.060 x edat) + (1.084 x circumferència abdominal) + (14.00 x requeriments de insulina) + triglicèrids. Un HH-score >242,4 va mostrar un 100% de sensibilitat i un 53% d’especificitat per el diagnòstic d’HH. La prevalença d’HH i de hipertensió arterial va augmentar en paral.lel a l’increment del quartil de la variable “triglicèrids + perímetre abdominal”. El pacients en el quartil superior van presentar una major resistència a la insulina, requeriments de insulina, circumferència de maluc, porcentatge de massa grassa, hemoglobina glucosilada (HbA1c), colesterol total i cLDL i nivells més baixos de cHDL i testosterona total. 6 pacients amb HH van rebre tractament amb UT i 7 amb placebo. A les 22 setmanes, es va observar un descens significatiu del colesterol total de 37.4 ± 27.5 mg/dL en el grup tractat amb UT comparat amb un increment de 13.2 ± 17.8 mg/dL en el grup amb placebo (P=0.005), també es va observar un descens significatiu del colesterol LDL de 30.2 ± 22.1 mg/dL en el grup amb UT comparat amb un increment de 10.5 ± 13.4 mg/dL en el grup placebo (P=0.004). Es va observar una reducció significativa del rati colesterol total /cHDL i dels triglicèrids en el grup tractat amb UT. No es van observar diferències en la sensibilitat a la insulina, el control metabòlic, l’antropometria ni la PA. Conclusions L’edat, la circumferència abdominal i els requeriments de insulina són predictors d’HH en pacients amb DM1. La presència de cintura hipertrigliceridèmica s’associa amb l’HH, un perfil metabòlic més desfavorable i factors de risc cardiovascular. El tractament amb UT en pacients amb DM1 i HH produeix una millora en el perfil lipídic.


Introduction The prevalence of hypogonadotropic hypogonadism (HH) in patients with type 2 diabetes mellitus is close to 17-30%. Testosterone replacement therapy in these patients has shown improvement in insulin sensitivity, glycemic control and cardiovascular risk factors. Few studies have examined its role in type 1 diabetes mellitus (T1DM). Aim To determine the prevalence of HH in patients with T1DM and associated risk factors. To assess the associations between the prevalence of hypertriglyceridemic waist and cardiovascular risk factors and HH. To evaluate the effectiveness of testosterone undecanoate (TU) in patients with T1DM and HH on insulin sensitivity, glycemic control, anthropometric parameters, blood pressure and lipid profile, as well as analyze changes in the International Index of Erectile Function. Methods A cross-sectional study was conducted in patients with age ≥ 18 years and diagnosis of autoimmune diabetes. To identify the hypertriglyceridemic waist phenotype, triglyceride levels (mg/dL) + abdominal circumference (cm) taken together was stratified into quartiles. We performed a randomized placebo-controlled multicenter study which included patients with autoimmune diabetes with total testosterone levels <10 nmol/L and/or free testosterone calculated using the Vermeulen formula < 225 pmol/L, with low/normal LH/FSH on two separate early-morning days. Patients were randomly assigned to TU treatment or placebo with the following dosing schedule: baseline, 6 weeks and 16 weeks. Results 181 patients were included. The prevalence of HH was 8.3% (95% confidence interval [CI]: 4.3-12.3%). Age (odds ratio [OR] 1.066 [95% IC: 1.002-1.134]), waist circumference (OR 1.112 [95% IC: 1.028-1.203]), and insulin requirements ([UI/Kg] x 10 [OR 1.486 {95% IC: 1.052-2.098}]) were independently associated with the presence of HH. The model that best predicted HH generated this formula: HH-score = (1.060 x age) + (1.084 x waist circumference) + (14.00 x insulin requirements) + triglycerides. An HH-score >242.4 showed 100% sensitivity and 53% specificity for HH diagnosis. And increased prevalence of HH and hypertension was found paralleling with an increase in triglyceride + waist circumference quartile. Patients in higher quartile showed and increased insulin resistance, insulin requirements, hip circumference, percentage of fat mass, glycosylated hemoglobin (HbA1c), total cholesterol, LDL cholesterol and lower levels of HDL cholesterol and total testosterone. 6 patients with HH recibed tretament with TU and 7 patients with placebo. At 22 weeks, the decrease in total cholesterol was 37.4 ± 27.5 mg/dL in the TU group compared with an increase of 13.2 ± 17.8 mg/dL in the placebo group (P=0.005), and LDL cholesterol concentration decreased 30.2 ± 22.1 mg/dL, compared with an increase of 10.5 ± 13.4 mg/dL in the placebo group (P=0.004). A greater and significant reduction in total cholesterol / HDL cholesterol ratio and triglycerides was observed in the TU group. No differences were observed regarding insulin sensitivity, HbA1c or basal glucose, anthropometric parameters or blood pressure. Conclusions Age, waist circumference and insulin requeriments predict HH in patients with T1DM. Hypertriglyceridemic waist is associated with HH, unfavorable metabolic profile and cardiovascular risk factors. Replacement therapy with TU in patients with T1DM and HH improves lipid profile.
