Impacto del grado de resección quirúrgica en la calidad de vida y la supervivencia en pacientes con gliomas. Resultados de un estudio prospectivo

Abstract

Los gliomas difusos representan los tumores primarios malignos más frecuentes del Sistema Nervioso Central. Tradicionalmente los gliomas grado III y IV o Glioblastoma (GBM) se denominan de alto grado (GAG) y los grados I y II de bajo grado (GBG). Se analizó el impacto del grado de la resección quirúrgica en la Calidad de Vida (QoL) y la supervivencia en los pacientes con gliomas en el Hospital México, Costa Rica, en un periodo de septiembre del 2009 a abril del 2015, el grado de resección se valoró en estudios digitales de resonancia magnética nuclear y tomografía computada a través de una medición volumétrica digital, antes y después de la cirugía. Se evaluó en 187 pacientes los test basales de QoL, The European Organization for Research and Treatment of Cancer Core QOL Questionnaire (EORTC QLQ-C30) y el Sickness Impact Profile (SIP), junto con las pruebas Mini Mental Status Examination y la Escala de Karnofsky (KPS). De los 187 pacientes que realizaron el test basal, 117 (62,6%) realizaron los cuestionarios postoperatorios del EORTC QLQ-C30 y el SIP a las 8 semanas. Se correlacionó la supervivencia de acuerdo al porcentaje de resección: 1-biopsia (< 10% de la lesión); 2-parcial (10 a < 90%) y 3- subtotal/completa (> 90 a 100%), y el volumen tumoral residual. El 58,8% de los pacientes fueron hombres, la edad media fue de 49,4 ± 15,1 años. Los pacientes con tumores grado IV fueron el grupo mayoritario (65,3%), seguido de los pacientes con gliomas grado II (20,1%); los GAG representaron el 77,2%. La KPS se identificó como la variable predictora mejor relacionada con las variables de QoL basal y un KPS bajo en el momento del diagnóstico se asoció, para toda la serie, con un riesgo de mayor deterioro en los tests postoperatorios en aspectos intelectuales/emotivos (Actividad Intelectual del SIP OR: 1,05; IC 95%: 1,02-1,08; p=0,007; Función Emocional del EORTC QLQ-C30 OR: 1,05; IC 95%: 1,01-1,09; p=0,038) y Fatiga del EORTC QLQ C-30 (OR: 1,25; IC 95%: 1,04-1,49; p=0,016). El impacto en la QoL basal del SIP y el EORTC QLQ-C30 en los GBG y GAG fue similar. La afectación de las funciones cognitivas en los test basales de QoL fue relevante, la categoría más comprometida del SIP fue la Actividad Intelectual (mediana 44,8 puntos; mín: 0,0; máx 100) para toda la serie. Las categorías Físicas fueron las menos comprometidas, la Dimensión Física del SIP presentó mediana de 23,1 puntos; mín: 0,0; máx: 87,8 para toda la serie y la Función Física del EORTC QLQ-C30 fue la menos comprometida en los pacientes con GBG (mediana 93,2 puntos; mín: 0,0; máx: 100) y la segunda en los GAG (mediana 73,3 puntos; mín: 0,0; máx: 100). Los pacientes con tumores en el hemisferio dominante presentaron mayor deterioro cognitivo y las lesiones en la región centromedial de los hemisferios cerebrales presentaron una importante afectación en la mayoría las categorías del SIP y el EORTC QLQ-C30. Las puntuaciones de la QoL mejoraron después de la cirugía en todas las variables evaluadas del SIP y EORTC QLQ-C30 cuando el porcentaje de resección tumoral fue subtotal/completa y empeoraron cuando fue una biopsia en todas las dimensiones, sobre todo en las categorías físicas. La supervivencia para toda la serie aumentó cuando el porcentaje de resección tumoral fue mayor (HR: 0,98; IC 95%: 0,97-0,99; p=0,008). En pacientes con GBM la resección tumoral superior al 90% o el volumen tumoral residual inferior a 2 cc impactó de manera positiva la supervivencia (Log-rank test: 17,8; p<0,001 y Log-rank test: 7,41; p=0,025 respetivamente). Se ratificó el beneficio para la supervivencia y la QoL en las resecciones tumorales amplias.


Diffuse gliomas represent the most common primary malignant tumors of the Central Nervous System. Traditionally grade III and IV gliomas or glioblastoma (GBM) are referred to high grade (HGG) and grade I and II low grade (LGG). The impact of the degree of surgical resection on Quality of Life (QoL) and survival in patients with gliomas at Hospital México, Costa Rica, from September 2009 to April 2015, was analyzed. The degree of resection was evaluated in digital studies of nuclear magnetic resonance and computed tomography through digital volumetric measurement, before and after surgery. The baseline QoL test was evaluated in 187 patients, The European Organization for Research and Treatment of Cancer Core QOL Questionnaire (EORTC QLQ-C30) and the Sickness Impact Profile (SIP), also the Mini Mental Status Test Examination and the Karnofsky Scale (KPS). Of the 187 patients who performed the baseline test, 117 (62.6%) performed the EORTC QLQ-C30 and SIP postoperative questionnaires at 8 weeks. Survival was correlated according to the percentage of resection: 1-biopsy (< 10% of the lesion); 2-partial (10 to < 90%) and 3-subtotal/complete (> 90 to 100%) and residual tumor volume. The 58.8% of the patients were men, mean age was 49.4 ± 15.1 years. Patients with grade IV tumors were the majority (65.3%), followed by patients with grade II gliomas (20.1%); the HGG accounted for 77.2%. The KPS was identified as the predictor variable best related to baseline QoL variables and a low KPS at the time of diagnosis was associated, for the whole series, with a greater risk of deterioration in postoperative tests in intellectual / emotive aspects (Intellectual Activity of SIP OR: 1.05; CI 95%: 1.02-1.08; p = 0.007; Emotional Function EORTC QLQ-C30 OR: 1.05; CI 95%: 1.01-1.09; p = 0.038) and Fatigue EORTC QLQ C-30 OR: 1.25; CI 95%: 1.04- 1.49; p = 0.016). The impact on baseline QoL of SIP and EORTC QLQ-C30 in LGG and HGG was very similar. The affectation of the cognitive functions in the basal QoL tests was relevant. The most committed category of the SIP was the Intellectual Activity of the Psychosocial Dimension, (44.8 points, min: 0.0, max 100) for the whole series. The Physical categories were the least compromised, the Physical Dimension of the SIP presented median 23.1 points (min: 0.0; max: 87.8) for the whole series and the physical function of EORTC QLQ-C30 was the least compromised in patients with LGG (median 93.2 points, min: 0.0, max: 100) and the second in HGG (median 73.3 points min: 0.0; max: 100). Patients with tumors in the dominant hemisphere had greater cognitive impairment and lesions in the centromedial region of the cerebral hemispheres were significantly affected in most SIP and EORTC QLQ-C30 categories. The QoL scores improved after surgery in all of the SIP and EORTC QLQ-C30 evaluated variables when the tumor resection percentage was subtotal/complete and worsened when a biopsy was performed les in all dimensions, especially in the physical categories. Survival for the whole series increased when the percentage of tumor resection was higher (HR: 0.98, 95% CI: 0.97-0.99, p = 0.008). In patients with GBM, tumor resection greater than 90% or residual tumor volume less than 2 cc positively impacted survival (Log-rank test: 17.8, p <0.001 and Log-rank test: 7.41, p = 0.025 respectively). The benefit of extensive tumor resections in survival and QoL was ratified.
