Necessitats de la població atesa a les unitats psiquiàtriques penitenciàries de Catalunya

Abstract

Aquesta tesi té com a objectiu augmentar el coneixement sobre les necessitats dels pacients atesos a les unitats psiquiàtriques penitenciàries (UPP). Per assolir aquest objectiu s’han dissenyat tres estudis. Una revisió sistemàtica dels estudis que avaluen les necessitats en aquesta població; un estudi descriptiu de les necessitats de les persones ateses en UPP i de les variables que s’associen a les necessitats i, finalment, un estudi que avalua el canvi d’aquestes necessitats al llarg de 12 mesos d’estada en les unitats de rehabilitació de les UPP. L’escala Camberwell Assessment of Needs-Forensic Version és l’eina que permet avaluar les necessitats d’aquesta població i que s’ha utilitzat en els tres estudis d’aquesta tesi. Els resultats d’aquesta tesi ens mostren que els pacients i els professionals identifiquen, generalment, les mateixes necessitats, tant totals com no cobertes. Les necessitats més prevalents dels pacients atesos en les UPP són les relacionades amb la simptomatologia psicòtica i les activitats diàries. S’ha constatat la relació existent entre el nombre de necessitats i el funcionament general i social dels pacients. Tanmateix, altres dominis, com la qualitat de vida i la consciència de malaltia tenen un paper important en les necessitats totals i no cobertes dels pacients. L’estada en unitats de rehabilitació, així com, les intervencions i l’aplicació de programes psicològics, educatius i d’infermeria, contribueixen a la millora de l’estat psicopatològic dels pacients, al seu funcionament social i general, així com a la disminució dels factors i el nivell de risc. Les limitacions d’aquesta tesi parteixen de la manca d’instruments específics per a la població psiquiàtrica penitenciària, així com per la dificultat de dur a terme estudis controlats degut a les característiques d’aquestes institucions i el nombre reduït de població. Aquesta tesi presenta per primera vegada, resultats sobre les necessitats i l’evolució de la població ingressada a les UPP catalanes i es pot considerar un punt de partida per a futures investigacions.

The aim of this thesis is to increase the knowledge about the needs of patients admitted in the psychiatric penitentiary units (UPP). To achieve this goal three studies have been designed. A systematic review of the studies that assess the needs in this population; a descriptive study of the needs of the people attended in the UPP and the variables that are associated to the needs and, finally, a study that assess the change of these needs during 12 months of stay in the rehabilitation units of the UPP. The Camberwell Assessment of Needs-Forensic Version scale is the instrument that permits the needs assessment of this population and it has been used in all three studies of this thesis. The results of this thesis show that patients and staff, generally, identify the same needs, both total and unmet. The most common needs of patients treated at UPP are those related to psychotic symptoms and daily activities. The relationship between the number of needs and the general and social functioning of the patients has been established. However, other domains, such as quality of life and disease awareness, play an important role in the total and unmet needs of patients. The stay in rehabilitation units, as well as the interventions and the application of psychological, educational and nursing programs, contribute to the improvement of the psychopathological state of the patients, their social and general functioning, as well as the decrease in factors and the level of risk. The limitations of this thesis are based on the lack of specific instruments for the penitentiary psychiatric population, as well as the difficulty of carrying out controlled studies due to the characteristics of these institutions and the small number of people. This thesis presents for the first time results on the needs and evolution of the population admitted to the Catalan UPP and can be considered a starting point for future research.
