Estudio longitudinal de los cambios en el funcionamiento cognitivo en adolescentes con anorexia nerviosa tras la recuperación nutricional

Abstract

La Anorexia Nerviosa (AN) es el trastorno de la conducta alimentaria que más se ha estudiado a nivel neuropsicológico, aunque la mayoría de estudios sobre las alteraciones cognitivas en la AN se han llevado a cabo en población adulta. Su investigación es mucho más escasa en población adolescente, edad en la que habitualmente tiene lugar el inicio del trastorno y que conlleva importantes repercusiones en el desarrollo personal, académico/laboral y social del individuo. En el presente trabajo se estudió el funcionamiento cognitivo de adolescentes (de 11 a 18 años) diagnosticados de AN con una corta duración del trastorno y en un momento agudo de la enfermedad, así como su evolución tras 6 meses de tratamiento y recuperación ponderal en comparación con un grupo control del mismo sexo, edad y nivel de estudios. Los resultados obtenidos fueron que el perfil neuropsicológico de los pacientes con AN en infrapeso presentaba un mayor enlentecimiento en el procesamiento de la información visual compleja. Aunque cuando analizamos los perfiles cognitivos observamos que no es igual en todos ellos, presentándose únicamente en un 30% de la muestra un perfil cognitivo deficitario. Las dificultades que presentan se encuentran en la memoria visual, velocidad psicomotora y curva de aprendizaje verbal. Este subgrupo se caracteriza además por presentar un Índice de Masa Corporal (IMC) más bajo y elevada presencia de ansiedad rasgo. En el estudio longitudinal una vez recuperado el peso se encontró que los pacientes mejoraro su perfil neuropsicológico general. Sin embargo, cuando comparamos aquellas pacientes que han recuperado el ciclo menstrual, con aquellas que todavía sufren amenorrea y el grupo control, observamos que las pacientes que aún no han reestablecido el ciclo menstrual presentan dificultades cognitivas en la memoria visual, en las habilidades visuoconstructivas, débil coherencia central y menor resistencia a la interferencia. Las pacientes con AN que han recuperado el ciclo menstrual no diferían en su funcionamiento cognitivo con respecto a los controles. En conclusión, los resultados nos sugieren que no hay importantes dificultades en el funcionamiento general cognitivo de todos los adolescentes con AN. Las alteraciones más significativas en el rendimiento neurocognitivo se encuentran cuando se relacionan con alguna variable clínica o biológica. Variables clínicas, como el IMC muy bajo o la amenorrea, asociadas a un peor funcionamiento cognitivo, podrían estar relacionadas con un peor curso y pronóstico de la enfermedad. Conocer el funcionamiento cognitivo de los adolescentes con AN nos permitirá conocer mejor el funcionamiento global de la enfermedad y desarrollar mejores intervenciones terapéuticas.


Anorexia Nervosa (AN) is the eating disorder that has been most studied at neuropsychological performance, although most studies on the cognitive impairment in AN have been conducted in the adult population. It has been less studied in adolescent subjects, age in which usually takes place the beginning of a disorder and that entails significant impact on the personal, academic and social development of the individual. In this work it was studied the cognitive performance of adolescents (aged 11 to 18) diagnosed with AN with a short duration of the disorder and acute moment of disease, as well as its evolution after 6 months of treatment and weight recovery when compared to a control group of same sex, age and educational level. The obtained results were that the neuropsychological profile of underweight patients with AN were slower in the processing of complex visual information. Although when we look at cognitive profiles we observe is not equal in all of them, appearing an impaired cognitive performance only at 30% of the sample. The inefficiencies that they present are visual memory, verbal learning curve and psychomotor speed. This subgroup is also characterized by a lower Body Mass Index (BMI) and presence of a high anxiety trait. In the longitudinal study when AN recovered weight, it was found that patients normalized general neuropsychological performance profile. However, when we compared those patients who have recovered the menstrual cycle, with those who still suffer from amenorrhea and with the control group, we observed that patients who have not yet re-established the menstrual cycle have cognitive inefficiencies in visual memory visual-constructive abilities, weak central coherence and lower resistance to interference. AN patients that have recovered the menstrual cycle do not differ in their cognitive performance with respect to the controls. In conclusion, the results suggest that there are not important difficulties in general cognitive performance of all adolescents with AN. The most significant inefficiencies in the neurocognitive performance are when they are related to some clinical or biological variable. Clinical variables, such as very low BMI or amenorrhea, associated with poorer cognitive functioning, could be related to a worse course and prognosis of the disease. To know the cognitive functioning of adolescents with AN will allow us to better understand the overall functioning of the disease and to develop better therapeutic interventions.
