Empoderament de les infermeres en entorns de simulació clínica interdisciplinària i a la realitat assistencial durant l'arribada de la COVID-19. Exploració del constructe i dels factors que l'influencien

Abstract

Objectius: La investigació cerca conèixer com es reflecteix l'empoderament de les infermeres en entorns de simulació clínica interdisciplinària i a la realitat assistencial durant la primera onada de la COVID-19, juntament amb els factors d’ambdós entorns que generen influència en el constructe. Metodologia: Estudi qualitatiu de tipus fenomenològic interpretatiu. Mostreig intencional fins a saturació de dades. Les dades s’obtenen mitjançant observació no participant en entorns de simulació; entrevistes semiestructurades a infermeres participants; observació participant en una unitat de semicrítics de nova creació durant l’arribada de la COVID-19 i mitjançant entrevistes a infermeres que en formen part. S’obtenen 35 codis, subsumits en 8 agrupacions i dos temes finals. Resultats: Es determina l’existència de factors que influencien l’empoderament de les infermeres. Es basen en l’existència d’un clima positiu dins l’equip; la percepció de seguretat relacionada amb l’entorn; la generació d’emocions; la identificació d’aspectes clau; i la possibilitat de millora individual i col·lectiva. Es detecten manifestacions d’empoderament de les professionals, vinculades a la introspecció i l’autoanàlisi; la consciència de l’impacte; i les percepcions positives. Conclusions: L’evidència de manifestacions d’empoderament de les infermeres a l’entorn de simulació contribueix a certificar la utilitat de la tècnica per a promoure’l. La detecció d’aquestes al context assistencial estudiat, en facilita la identificació en altres entorns. La determinació de factors que influencien l’empoderament a l’entorn assistencial contribueix a determinar quins aspectes afavoreixen el constructe i quins l’impedeixen, mentre que el fet d’evidenciar aquests factors a l’entorn de simulació permet entreveure en quins aspectes de la metodologia cal incidir per tal de fomentar l’empoderament d’aquestes professionals mitjançant la seva utilització.

Objective: The research aims to find out how the nurses’ empowerment is reflected in interdisciplinary clinical simulation environments and in the real context during the first wave of COVID-19, together with the factors of both environments that influence the construct. Method: Qualitative phenomenological interpretative study. Purposive sampling until data saturation. The data is obtained through non-participant observation in simulation sessions; semi-structured interviews and participant observation in a newly created semi-critical care unit during the arrival of COVID-19. 35 codes are obtained, distributed into 8 groups and two final subjects. Results: It has been possible to determine the existence of factors that influence the nurses’ empowerment. These are based on the existence of a positive atmosphere within the team; on the perceived security related to the environment; on the existence of emotions; on the identification of key aspects and on the possibility of individual and collective improvement. Likewise, manifestations of nursing empowerment have been detected, linked to introspection and self-analysis; awareness of the impact and positive perceptions. Conclusions: The evidence of manifestations of nurses’ empowerment in the simulation environment contributes to certify the usefulness of the technique to promote it, while the detection of these in the real context facilitates their identification in other realities. The determination of factors that influence empowerment in the real environment contributes to determine which aspects improve it and which of them difficult it. Evidencing these factors in the simulation context enables the identification of the aspects of the methodology that must be intensified to promote the empowerment of these professionals through its use.
