Abstract

Introducción: El Trasplante de Células Progenitores Hematopoyéticas (TPH) es una modalidad terapéutica muy eficaz en el tratamiento de muchas de las neoplasias onco-hematológicas y algunas hemopatias congénitas e inmunodeficiencias primarias. Entre los factores que pueden interferir en la reconstitución inmunológica post-TPH se encuentran: la fuente de progenitores hematopoyéticos (médula ósea (MO), sangre de cordón umbilical (SCU), sangre periférica (SP)), el tipo de trasplante (autólogo o alogénico), la situación clínica del paciente antes del trasplante, la enfermedad de base (neoplásica o no), el tratamiento de condicionamiento utilizado, la profilaxis contra la Enfermedad Injerto Contra Huésped (EICH), y el desarrollo o no de la EICH (aguda o crónica) y su terapia.<br/><br/>Objetivos: Evaluar la reconstitución inmunológica en las distintas modalidades de trasplantes de células progenitoras hematopoyéticas (TPH) en niños y estudiar si esta depende de la enfermedad de base, del tipo de trasplante realizado, de la fuente de los progenitores hematopoyéticos y del grado de la EICH.<br/><br/>Material y Métodos: Para ello se estudió la reconstitución de las subpoblaciones linfocitarias T, B y NK, de la función T y la respuesta humoral (IgA, IgG, IgM), de la producción de anticuerpos naturales (antiestreptolisina O) y de la respuesta a los antígenos vacunales de la hepatitis B (anti-HBs) y del tétanos (anti-toxoide tetanico) en 61 niños receptores de TPH, en cualquiera de sus modalidades, a los 1, 3, 6, 12, 18 y 24 meses post trasplante en la Unidad de Trasplante de Médula Ósea del Hospital Materno Infantil Vall d´Hebron de Barcelona y se analizaron las diferencias en la reconstitución inmunológica entre: a) los tres tipos de fuentes: MO, SP y SCU; b) trasplantes autólogos y alogénicos; c) pacientes con enfermedades neoplásicas y no neoplásicas; d) y según la severidad de la enfermedad injerto contra huésped aguda y crónica; e) y el grado de respuesta a las vacunas (tétanos y hepatitis) en relación a los niveles de inmunoglobulinas G y la presencia de anticuerpos naturales.<br/><br/>Resultados y Conclusiones: La SCU y la SP son las fuentes de PH que presentan una reconstitución inmunológica más rápida de las subpoblaciones linfocitarias respecto a MO. No hay diferencias significativas en la recuperación funcional de los linfocitos T según los tipos de progenitores utilizados, tipo de trasplante empleado o enfermedad de base del niño, si bien la recuperación es más lenta en los receptores de SCU y en los receptores de TPH alogénicos. Los grados superiores de EICH Aguda ( > III) y la EICH Crónica conllevan a un retraso en la reconstitución inmunológica, debido a los tratamientos inmunosupresores por largos periodos de tiempo. La reconstitución inmunológica de los linfocitos B CD19+ es más lenta que la de las subpoblaciones linfocitarias T. El cociente CD4+/CD8+ aparece invertido a lo largo del primer año del estudio por una reducción de los linfocitos T CD4+. Las células citotóxicas naturales (NK) tienen una recuperación muy temprana tanto en relación a las fuentes utilizadas (MO, SCU y SP), como con los tipos de trasplante y la enfermedad de base. La negativización de los anticuerpos anti-hepatitis B ocurre en una mayor proporción que la de los anticuerpos anti-tétanos, y la respuesta post vacunación en conjunto fue menor en porcentaje que la anti-tétanos. La producción de ASTO en los niños mayores de 2 años, fue mayor en los niños que recibieron TPH de MO y de SP respecto a los que recibieron TPH de SCU y en los niños que recibieron TPH autólogo, respecto al alogénico.


Introduction: Haematopoietic stem cell (HSC) transplantation is a highly efficacious therapy for many onco-haematologic neoplasias and some congenital primary immunodeficiencies and metabolic disorders. Factors which may interfere in post-HSC immunologic reconstitution include: the HSC source (bone marrow (BM), umbilical cord blood (UCB), peripheral blood (PB), type of transplant (autologous or allogeneic), clinical status of the patient prior to transplant, the underlying disease (neoplastic or not), the conditioning regimen used, graft vs. host disease (GVHD) prophylaxis and the development or not of GVHD (acute or chronic) and its therapy.<br/><br/>Objectives: To evaluate the immunologic reconstitution in the different modes of transplants in children and study whether this depends on the underlying disease, type of transplant performed, HSC source and degree of GVHD.<br/><br/>Material and Methods: We studied the reconstitution of lymphocyte T, B and natural killer (NK) subpopulations, T function and humoral response (IgA, IgG, IgM), natural antibody production (antistreptolysine O - ASTO) and response to hepatitis B (HBs) and tetanus (anti-toxoid tetanic) vaccines in 61 recipients in the different HSC transplant modalities, at 1, 3, 6, 12, 18 and 24 months post-transplant at the Bone Marrow Transplant Unit of the Hospital Materno-Infantil Vall'Hebron, Barcelona. We analysed the differences in immunologic reconstitution between: a) the three types of sources, BM, PB and UCB; b) autologous and allogeneic transplants; c) patients with neoplastic and non-neoplastic diseases; d) presence or absence of severe or chronic GVHD. The degree of response to vaccines (tetanus and hepatitis B) was studied in relation to immunoglobulin G levels and presence of natural antigens.<br/><br/>Results and Conclusions: UCB and PB are the HSC sources which present the most rapid immunologic reconstitution of lymphocytic subpopulations with respect to BM. No significant differences were observed in the functional recovery of T lymphocytes according to the types of stem cells used, type of transplant performed and the child's underlying disease, although recovery was slower in UCB recipients compared with other sources and in allogeneic HSC recipients compared with autologous recipients. Higher grades (> III) of acute GVHD and chronic extensive GVHD correlated with a delay in immunologic reconstitution due to long-term immunosuppressor treatment. The reconstitution of B lymphocytes was slower than that of T lymphocyte subpopulations. The CD4+/CD8+ ratio appeared inverted throughout the first year of the study due to a reduction in CD4+ T lymphocytes. NK cells always had an early recovery regardless of source used (BM, UCB, PB), type of treatment or underlying disease. Anti-hepatitis B antibody disappeared post-transplant in a greater proportion than anti-tetanus antibodies; furthermore, the post-vaccine response was better after the anti-tetanus vaccine.
