Abstract

L’actual projecte de tesi planteja un treball innovador a través de la metodologia observacional aplicada amb estudis descriptius amb pacients amb una lesió medul·lar adquirida (d’origen traumàtic o mèdic) amb dolor. Els pacients se sotmeten a un protocol clínic d’estimulació cerebral no invasiva (tDCS) i realitat virtual (RV) pel maneig del dolor neuropàtic. A partir de la revisió bibliogràfica i de l’experiència clínica es descriu com a hipòtesi general que les variables personals, neurobiològiques i fisiopatològiques condicionen l’efecte i la resposta dels tractaments realitzats amb l’estimulació transcraneal per corrent directe, provocant diferències individuals. Així doncs, l’objectiu general d’aquest projecte de tesi doctoral és el d’aprofundir en el coneixement de les variables predictives i explicatives de la millora clínica en pacients lesionats medul·lars amb dolor neuropàtic que realitzin tractament d’estimulació cerebral no invasiva (tDCS), combinat amb tècniques de realitat virtual, per optimitzar el pronòstic de les intervencions. Es presenten, com a resultats d’investigació, tres estudis que segueixen un protocol clínic específic amb la intenció de descriure el protocol utilitzat, validar la presència d’efectes secundaris, identificar variables descriptives, estudiar els diferents perfils sensorials i analitzar mostres específiques de pacients amb lesió medul·lar i dolor neuropàtic. Les principals conclusions de la tesi són: i) La teràpia neuromoduladora tDCS, juntament amb la RV, desencadena efectes beneficiosos per pacients amb dolor neuropàtic després d’una lesió medul·lar; ii) La variable de cronicitat no ha actuat com a predictora de resposta davant del tractament d’estimulació cortical tDCS i RV; iii) No existeix un fenotip clínic sensorial que respongui més eficaçment en l’aplicació del protocol d’estimulació; i per últim iv) No existeixen suficients estudis en la literatura que validin uns mateixos paràmetres de seguretat, variabilitat, fiabilitat i interferències en les respostes.


The current thesis project raises an innovative study through the observational methodology applied at descriptive studies with patients with a spinal cord injury (either traumatic or medical origin) and pain. Patients have been exposed to a non-invasive brain stimulation protocol (combined with virtual reality) for the management of neuropathic pain. Based on the bibliographic review and clinical experience, it is described as a general hypothesis that personal, neurobiological and pathophysiological variables can modulate the effect and treatment responses performed by transcranial direct current stimulation (tDCS), causing individual differences. Therefore, the main objective of this doctoral thesis is to deepen in the knowledge of the predictive and explanatory variables of the clinical improvement in spinal cord injury (SCI) patients with neuropathic pain that carry out treatment of non-invasive brain stimulation (tDCS), combined with virtual reality techniques, to optimize the prognosis of interventions. As a result of research, three studies are presented that follow a specific clinical protocol with the intention of describing its protocol, validating the presence of side effects, identifying descriptive variables, studying the different sensory profiles and analyzing specific samples of patients with spinal cord injury and neuropathic pain. The main conclusions of the thesis are: i) tDCS neuromodulation therapy, together with RV, triggers beneficial effects for patients with neuropathic pain after a spinal cord injury; ii) The chronicity has not acted as a predictor of response to cortical stimulation treatment; iii) There is no sensory clinical phenotype that responds more effectively in the application of the stimulation protocol; and finally iv) There are not enough studies in the literature that validate the same parameters of security, variability, reliability and interference in the responses.
