Neumonitis por hipersensibilidad (NH) es un término que abarca un grupo
heterogéneo de enfermedades resultantes de una reacción inflamatoria pulmonar
de causa inmunológica, cuyos mecanismos no son bien conocidos, en respuesta
a una amplia variedad de antígenos que pueden provocar diversos grados de
inflamación y desestructuración del parénquima pulmonar.
La prevalencia e incidencia de la NH dependen de factores tales como las
condiciones geográficas o la intensidad de la inhalación; se estima que la NH
representa alrededor del 15% de las enfermedades pulmonares intersticiales
difusas (EPID).
Respecto al diagnóstico, actualmente no existen unos criterios validados, por lo
que el diagnóstico se basa en diferentes aspectos como una alta sospecha
clínica, la demostración de un antígeno responsable o los hallazgos radiológicos.
La NH es una entidad potencialmente grave, con una mortalidad a los 5 años de
hasta 27-29% de los pacientes. Des del punto de vista farmacológico en la
actualidad se usan diferentes estrategias inmunosupresoras y, cuando la NH
progresa, el trasplante pulmonar si no existen contraindicaciones.
Así, en este contexto de ausencia de una prueba de referencia para el diagnóstico
de la NH por un lado y de dudas sobre la inmunopatogenia de la NH por otro, en
la presente tesis se muestran los dos estudios realizados con el objetivo de
establecer la rentabilidad de la prueba de provocación bronquial específica
(PPBE) en el diagnóstico de la NH y el diseño de dos modelos animales de NH
(agudo y crónico) con el fin de analizar los diferentes mecanismos fisiopatológicos
en función de la forma de presentación y poder determinar si se trata de
diferentes estadios de la enfermedad.
Para poder analizar la rentabilidad de la PPBE, se realizó un estudio retrospectivo
con todos aquellos pacientes con sospecha de NH a los que se realizó la prueba
de provocación en nuestro centro (Hospital Vall d’Hebron) entre 1995 y 2010; tras
la aplicación del protocolo, se incluyeron un total de 113 sujetos. Se analizaron
datos de función pulmonar, de laboratorio, de imagen, del agente causal

sospechado y del resultado de la PPBE. Se determinó que la PPBE presentaba
una sensibilidad y especificidad del 72.7% y 84% respectivamente cuando no se
tenía en cuenta el agente etiológico; estos resultados mejoraban si se analizaban
únicamente las PPBE a aves y hongos, siendo respectivamente del 85.1% y
86.2%. Se observó que únicamente el 8% de los pacientes presentaron
reacciones graves transitorias, sin requerir ingreso hospitalario.
Por otro lado, con el fin de analizar los diferentes mecanismos fisiopatológicos
implicados en la NH, se realizó un estudio experimental con el diseño de dos
modelos murinos para NH aviar: un modelo agudo y otro crónico. Se realizó
análisis de la función pulmonar, las IgG en suero, la celularidad del lavado
broncoalveolar (LBA), las citoquinas/quimiocinas presentes en el LBA y la
histología pulmonar. Se observó una disminución de la TLC más marcada en el
grupo de NH crónica, sin observar recuperación con el tiempo. Las IgG se
detectaron en ambos grupos, siendo su concentración mayor en el grupo crónico.
El número de células totales en el LBA fue más elevado en el grupo NH crónica;
en este grupo se observó valores más altos de neutrófilos y linfocitos que en el
grupo de NH aguda; en la NH aguda se observaron eosinófilos no presentes en el
grupo crónico. Las citoquinas analizadas se observaron en ambos grupos, a
excepción del IFNg que no se detectó y la IL-4 que únicamente se detectó en el
grupo agudo, 24h después de la última instilación nasal. El estudio histológico
mostró cambios compatibles con NH tanto en el grupo agudo como crónico.
En conclusión, la PPBE no sólo es una herramienta útil en el diagnóstico de las
NH sino que también es segura. Los neutrófilos y la respuesta TH17 tienen un
papel importante en el desarrollo de la enfermedad y posiblemente, haya una
evolución de TH1 (NH aguda) a TH2 (NH crónica) ante la persistencia de la
exposición antigénica. Este hecho reforzaría la hipótesis de que las diferentes
formas de presentación pueden ser estadios evolutivos de la enfermedad.

Hypersensitivity pneumonitis (HP) is a term encompassing a heterogeneous group
of diseases resulting from an inflammatory pulmonary reaction of immunological
cause, whose mechanisms are not well known, in response to a wide variety of
antigens that can provoke varying degrees of inflammation and destructuring of
the lung parenchyma. The prevalence and incidence of HP depend on factors
such as geographic conditions or intensity of inhalation, estimated to be around
15% of diffuse interstitial lung disease. A great number of antigenic sources have
been described as etiologic agents, being the Bird fancier’s lung (BFL) one of the
most frequently HP in our setting.
Regarding diagnosis, currently there are no validated criteria, so the diagnosis is
based on different aspects such as a high clinical suspicion, the demonstration of
a responsible antigen or radiological findings.
It is a potentially serious entity since it can evolve to pulmonary fibrosis and
chronic respiratory failure, with mortality at 5 years of up to 27-29% of patients.
Currently, HP’s treatment consists in a differents immunosuppressive strategies
and, when HP progresses, pulmonary transplantation if there are no
contraindications.
In this context, with lack of a gold standard test for the diagnosis of HP and doubts
about the immunopathogenesis of HP, in this thesis show two studies conducted
with the objective of establishing the diagnostic yield of specific inhalation
challenge (SIC) in HP and the creation of two animal models of HP (acute and
chronic) to analyze the different physiopathological mechanisms depending on the
form of presentation of the disease and determine if the different forms of HP
presentation may correspond to different stages of the same disease.
For establish the diagnostic yield of SIC, a retrospective study was performed with
all patients with suspected HP who underwent the inhalation challenge in our
center (Hospital Vall d'Hebron) between 1995 and 2010; according to the
application of the protocol, a total of 113 subjects are included. Information on
pulmonary function, laboratory data, imaging tests, suspected causal agent and
SIC result was obtained. The SIC had a sensitivity and specificity of 72.7% and

84% respectively when the etiological agent was not taken into account; these
results improved if only SIC for avian and fungi agents were analyzed, with a
sensitivity and specificity of 85.1% and 86.2% respectively. It was observed that
only 8% of the patients presented severe transient reactions, without requiring
hospital admission.
On the other hand, to analyze the different pathophysiological mechanisms
involved in HP, an experimental study was carried out with the creation of murine
models for avian HP: one acute and one chronic model. It analysed of pulmonary
function, IgG, bronchoalveolar lavage cellularity (BAL), cytokines / chemokines
present in BAL and lung histology. A most decrease in the TLC was observed in
the chronic HP group, without observing recovery with time. The IgG were
detected in both groups, the concentration was higher in the chronic group. The
number of total cells in BAL was higher in the chronic HP group; in this group,
higher values of neutrophils and lymphocytes were observed in comparison with
acute HP group; in the acute HP group were observed eosinophils. The analyzed
cytokines were observed in both groups, with the exception of IFNg that was not
detected and IL-4 that was only detected in the acute group, 24h after the last
nasal instillation. The histological study showed changes compatible with HP in the
acute and chronic groups.
In conclusion, the SIC is a useful tool in the diagnosis of HP and it is also safe.
Neutrophils and the TH17 response have an important role in the development of
the disease and that there is possibly an evolution from TH1 (acute HP) to TH2
(chronic HP) due the persistence of antigenic exposure. This fact would strengthen
the hypothesis that the different forms of HP presentation may correspond to
different stages of the same disease.
