Abstract

Introducció La comparació dels resultats perinatals en funció de si la transferència embrionària (TE) s’ha realitzat en fresc o congelat (TEC) és un aspecte de la medicina reproductiva que ha donat lloc a múltiples debats. La majoria d’experts afirmen que la TEC s’associa a millors resultats perinatals tals com menor risc de baix pes al naixement (BPN), part pre-terme o petit per edat gestacional (PEG). En termes generals, dues són les hipòtesis que s’han postulat per tal d’explicar les diferències observades entre els grups de TE fresc o TEC: teoria de l’hiperestrogenisme i teoria del “weak embryo”. Aquest estudi té com a finalitat determinar si els resultats perinatals es veuen afectats per la vitrificació i/o per l’hiperestimulació ovàrica controlada (HOC). Per assolir aquest objectiu, s’han analitzat dues poblacions: dones que utilitzen òvuls propis i que, per tant, en algun moment s’han sotmès a HOC per finalment aconseguir els embrions i dones que són receptores d’un programa d’ovodonació, per consegüent, no sotmeses a processos d’HOC. Material i mètodes S’ha dissenyat un estudi de cohorts retrospectives que inclou dones que realitzaren una Fecundació In Vitro (FIV) a Catalunya entre els anys 2008 i 2012, utilitzant òvuls autòlegs o de donació. Ambdues poblacions s’han classificat segons els tipus de TE portada a terme, TE en fresc o TEC, per tal d’avaluar les possibles diferències en els resultats perinatals entre els grups. Només s’han inclòs aquelles pacients que finalment van aconseguir una gestació única i amb resultat de part més enllà de la setmana 24 de gestació. La variable principal és pes al naixement (PN). Les variables secundaries són edat gestacional al naixement (EG), PEG, via del part i mortalitat perinatal. També s’han descrit les següents característiques basals: edat materna, anys d’esterilitat, origen de l’esperma i nombre total d’embrions transferits. Resultats A la cohort d’òvuls autòlegs, els nounats provinents de TE en fresc tenen un menor PN que els del grup de TEC; igualment s’ha observat que els primers tenen també un major risc de ser PEG. Per contra, al grup de receptores d’ovodonació, la mitjana de PN no presenta diferències entre grups i tampoc ho fa el risc de PEG. L’EG al naixement també presenta un patró similar en quant a resultats; major percentatge global d’infants nascuts pre-terme al grup de fresc que al de TEC de les dones que fan servir òvuls propis i no diferències a les receptores. La variable via del part mostra una major taxa de part instrumentat i cesàries al grup de TEC d’autòlegs i absència de diferències significatives a les receptores. Finalment, no s’han observat diferències estadísticament significatives en la mortalitat perinatal entre les cohorts, ni a les dones que utilitzen òvuls propis ni al grup de receptores d’ovodonació. Discussió i conclusions El fet d’utilitzar un programa d’ovodonació ha permès aïllar el possible efecte deleteri que la vitrificació i desvitrificació podrien tenir sobre els embrions, i en conseqüència sobre els resultats perinatals, de l’efecte hormonal que els cicles d’HOC tenen sobre l’endometri i la gestació resultant. El treball que es presenta suggereix que les diferències entre TE en fresc i TEC que ja s’havien descrit en estudis previs en quant a PN, EG i altres resultats perinatals, estan més relacionades amb l’efecte perjudicial que té la HOC sobre l’endometri que amb la selecció embrionària que es pogués produir durant els processos de vitrificació.


Introduction Several studies have been developed to compare perinatal outcomes after fresh embryo transfer (ET) and frozen-thawed ET (FET). Most of them have reported that FET is associated with improved perinatal outcomes, such as a lower risk of low birth weight (LBW), preterm birth and being small for gestational age (SGA). In general, two hypotheses have been suggested to explain these differences observed between groups: the hyperstimulation theory and the “weak embryo” theory. The aim of this study is to ascertain whether perinatal outcomes are affected by vitrification and/or by controlled ovarian hyperstimulation (COH). To achieve this goal, two populations have been analyzed: women using autologous eggs who were exposed to COH to obtain the embryos, and women who used donor eggs and did not undergo hyperstimulation processes. Material and methods A register-based cohort study which includes women undergoing In Vitro Fertilization (IVF) in Catalonia between 2008 and 2012, using autologous or donated eggs, is designed. Both populations are classified according to the type of ET performed, fresh ET versus FET, in order to examine possible differences in perinatal results. Only women who had a singleton pregnancy delivered from the 24th week onwards are included. The primary outcome is birthweight (BW). Secondary outcomes include gestational age at delivery, being SGA, mode of delivery and perinatal mortality. Baseline characteristics such as maternal age, years of infertility, sperm source, and number of transferred embryos are also described. Results In the autologous egg population, newborns from fresh ET group have lower BW than FET group; we also observe that the first aforementioned group have a higher risk of being SGA. In contrast, among egg donor recipients undergoing ET, mean BW do not differ between groups and neither does the risk of SGA. A similar pattern of results is found regarding to the gestational age at delivery; higher percentage of infants born pre-term in the fresh ET group than in the FET group of the autologous egg population and no differences in the egg donor recipients. The mode of delivery shows a higher rate of instrumental delivery and cesarean delivery in the FET group of the autologous and no differences in that outcome among the egg-recipient group. Finally, we observe no statistically significant differences in perinatal mortality between groups, either in the autologous egg population or in the recipient group. Discussion and Conclusions The fact of using an egg-donation program allowed us to isolate the possible detrimental effect of vitrification and devitrification on embryos, and consequently on the perinatal outcomes, from the hormonal effect that hyperstimulated cycles have on the endometrium. Our results suggest that previously reported differences in BW, gestational age at delivery, and other perinatal outcomes after fresh ET and FET are more likely to be related to the detrimental effects of hyperstimulation on the endometrium during COH than to the embryonic selection effect of the vitrification process.
