Avances en la investigación sobre el trastorno de personalidad límite. Estudio de subtipos y su respuesta al tratamiento

Abstract

El diagnóstico de Trastorno Límite de Personalidad (TLP) permite hasta 151 combinaciones según su sintomatología. Dada esta gran variabilidad, se viene investigando en la existencia de subtipos de TLP con el fin de elaborar tratamientos más eficaces. Este trabajo persigue 1) comprobar si una muestra de 67 personas con TLP puede dividirse en subtipos según la gravedad de sus síntomas; y 2) si esos subtipos responden diferente al tratamiento de 6 meses recibido. Se obtuvieron 2 clústers: 1) “Gravemente afectados”, con mayores puntuaciones en la mayoría de las variables estudiadas; y 2) “Levemente afectados”, con menores puntuaciones en dichas variables. En efecto, parece haber una respuesta distinta al tratamiento en función del subtipo. El clúster “Gravemente afectados” mejoraron tras el tratamiento, mientras que el clúster “Levemente afectados” empeoró. Se discuten las implicaciones clínicas de esta distinta respuesta a la intervención en la dirección de tratamientos personalizados

The diagnosis of Borderline Personality Disorder (BPD) allows up to 151 combinations according to its symptomatology. Given this great variability, the existence of subtypes of BPD has been investigated in order to develop more effective treatments. This work aims to 1) test whether a sample of 67 people with BPD can be divided into subtypes according to the severity of their symptoms; and 2) whether these subtypes respond differently to the 6-month treatment received. Two clusters were obtained: 1) "Severely affected", with higher scores on most of the variables studied; and 2) "Slightly affected", with lower scores on these variables. Indeed, there seems to be a different response to treatment depending on the subtype. The "Severely affected" cluster improved after treatment, while the "Slightly affected" cluster worsened. The clinical implications of this different response to intervention in the direction of personalized treatments are discussed.
