Comportamientos de riesgo para la salud y calidad de vida en adolescentes escolarizados

Abstract

The main objective of this research is to determine the association of different Health Risk Behaviours (HRB's) with the Health-Related Quality of Life (HRQoL) in a sample of 550 adolescents attending school, aged 15 to 18 years, enrolled in a secondary school in Valparaíso, Chile. The KIDSCREEN-52 Questionnaire was used to evaluate HRQoL and the World School Health Survey (EMSE) to identify HRB's. The KIDSCREEN-52 dimensions scores were calculated using the Rasch analysis, transformed to T values and standardized to M=50, SD=10. Comparisons of Rasch scores, bivariate and multiple logistic regression analyses were made to study the association of the dimensions of the HRQoL of KIDSCREEN-52 with the HRB's adjusted for age and gender. The data analysis was performed using the STATA v12 statistical package. This study was approved by the Bioethics Committee of the Playa Ancha University. Informed consent was requested from parents or legal guardians, and participating adolescents too. The comparisons of the Rasch scores showed higher punctuations in men than in women, and as the age increased, a decrease in the school environment dimension score was observed. Multiple logistic regression analyses indicate that psychoactive substance use, physical inactivity and suicidal behaviours are negatively related to most dimensions of HRQoL. Obesity was related to a worse self-perception and being victim of intimidation was related to a worse psychological well-being and social acceptance. The lack of psychosocial protective factors (parental regulation and control, and social support in school) was negatively related to most dimensions of HRQoL.

Aquesta recerca té com a objectiu principal determinar l’associació dels diferents Comportaments de Risc per a la Salut (CRS) amb la Qualitat de Vida Relacionada amb la Salut (QVRS) en una mostra de 550 adolescents escolaritzats de 15 a 18 anys matriculats a un institut d’educació secundària de Valparaíso, Xile. S’ha utilitzat el qüestionari KIDSCREEN-52 per avaluar la QVRS i la Encuesta Mundial de Salud Escolar (EMSE) per identificar els CRS. Les puntuacions de les dimensions del KIDSCREEN-52 s’han calculat emprant l’anàlisi Rasch, transformant a valors T, i estandarditzant a una M=50, DT=10. S’han realitzat comparacions de puntuacions Rasch, análisis bivariades i de regressió logística múltiple per estudiar l’associació de les dimensions de la QVRS del KIDSCREEN-52 amb els CRS ajustats per edat i sexe. L’anàlisi de les dades s’ha realitzat emprant el paquet estadístic STATA v12. Aquest estudi va ser aprovat pel Comité de Bioética de la Universidad de Playa Ancha. Es va sol·licitar consentiment informat a pares o tutors i als adolescents participants. Les comparacions de les puntuacions Rasch han mostrat puntuacions més elevades en homes que en dones, i a mesura que ha augmentat l’edat s’ha observat una disminució en la puntuació de la dimensió entorn escolar. Les análisis de regressió logística múltiple han indicat que el consum de substàncies psicoactives, la inactivitat física i els comportaments suïcides es relacionen negativament amb la majoria de les dimensions de la QVRS. L’obesitat s’ha relacionat amb una pitjor autopercepció, i ser víctimes d’intimidació s’ha relacionat amb un pitjor benestar psicològic i acceptació social. La manca de factors protectors psicosocials (regulació i control patern i el suport social a l’escola) s’han relacionat negativament amb la majoria de les dimensions de la QVRS.
