Abstract

Las Inmunodeficiencias Primarias (IDPs) son enfermedades congénitas del sistema inmune que tienen en común una susceptibilidad incrementada a las infecciones. Se han descrito más de 200 entidades englobadas bajo esta definición. Algunas de ellas tienen una herencia ligada al X, lo que les confiere ciertas peculiaridades. En esta Tesis presentamos los resultados concernientes a 28 pacientes, casi todos varones, con sospecha de IDP en base a una elevada frecuencia y severidad de infecciones. A cada uno de ellos se les evaluó inmunológicamente, lo que permitió, junto con las manifestaciones clínicas y los antecedentes familiares, establecer un diagnóstico presuntivo de IDP. A partir de aquí, realizamos estudios funcionales, moleculares y genéticos específicos para la IDP sospechada. Estos análisis permitieron identificar la causa genética de la enfermedad, así como ver el grado de afectación de la proteína mutada. Además, se estudió la presencia de la mutación en la familia para determinar portadoras de la patología y posibles pacientes no diagnosticados. En paralelo a estos estudios, los pacientes fueron tratados, siendo muchos de ellos sometidos a un trasplante de progenitores hematopoyéticos (TPH). En este trabajo también se evaluó la recuperación post-TPH repitiendo algunas de las pruebas realizadas durante el diagnóstico. En primer lugar, se estudiaron 6 pacientes con sospecha de Inmunodeficiencia Severa Combinada (SCID). Tras el análisis inmunológico y funcional se analizaron los genes candidatos y se establecieron las mutaciones responsables. Dos de ellos presentaban la forma ligada al X (X-SCID), con alteraciones en IL-2GR: p.Lys120fsX46 y p.Arg222Cys. El caso clínico 3 resultó tener una deficiencia en JAK-3 con una mutación no descrita (p.Thr456fsX520). El caso clínico 4 se diagnosticó de Disgenesia Reticular tras encontrar una mutación en AK2 (c.A1T). Aunque los casos clínicos 5 y 6 presentaban manifestaciones clínicas bastante diferentes, establecimos que ambos hermanos padecían un SCID-Omenn causado por la mutación c.631delT en RAG-2. En el análisis de los dos hermanos con sospecha de Síndrome de hiper-IgM ligado al X encontramos una gran deleción que impedía la expresión de CD40L. Asimismo, estudiamos dos pacientes con Agammaglobulinemia ligada al X: el caso clínico 9 mostró la mutación c.557dupA, mientras que el caso clínico 10, la p.Arg288Gln. A los 4 pacientes con sospecha de Síndrome de Wiskott-Aldrich se les encontró una mutación en el gen WAS: IVS6+5G>A, IVS3+2-3insT, p.Arg34STOP y IVS8+1G>A. El estudio del gen WAS también nos permitió establecer el diagnóstico de Neutropenia ligada al X en el caso clínico 15 (p.Ile294Thr). Se estudiaron 4 pacientes de 2 familias distintas con posible Síndrome Linfoproliferativo ligado al X. Una de las familias presentaba la mutación p.Val102Gly en el gen SH2D1A en los tres hermanos, a pesar de que el menor no había presentado sintomatología. En el otro paciente hallamos una deleción completa del gen. La prueba para la determinación de la capacidad oxidativa de los granulocitos nos permitió diagnosticar a 8 pacientes de Enfermedad Granulomatosa Crónica. Posteriormente, analizamos los genes responsables hasta encontrar el defecto genético. Los casos clínicos 20 y 21 presentaron mutaciones en el gen CYBB: p.Cys59Arg y p.Lys247fsX7, respectivamente. Los casos clínicos 22, 23 y 24 son 3 hermanos a los que les determinamos la presencia de la recombinación homóloga entre el gen NCF1 y su pseudogen. El análisis del caso clínico 25 mostró que era un heterocigoto compuesto para dos mutaciones en NCF2: p.Lys161STOP y una duplicación génica que incluye los exones 9 y 10. Mientras que los casos clínicos 26 y 27 también revelaron una alteración en NCF2, pero en este caso en homocigosis (p.Arg77STOP). Por último, el estudio del caso clínico 28 mostró la mutación más prevalente responsable de Incontinencia Pigmenti.


Primary Immunodeficiencies (PIDs) are congenital disorders of immune system characterized by increased susceptibility to infections. More than 200 entities have been included under this definition. Some of them have an X-linked inheritance, giving them certain peculiarities. In this Thesis, we presented the results of 28 patients, almost all males, with a suspicion of PID due to the higher frequency and severity of infections. First, we evaluated them immunologically. These data together with clinical manifestations and familial antecedents allowed us to establish a presumptive diagnosis of PID. Consequently, we performed specific functional, molecular and genetic studies to the suspected PID. These analyses allowed identifying the genetic cause of the disease and also establish the degree of affectation of the mutant protein. Moreover, we studied the presence of the mutation in their families to identify carriers of the disease and not diagnosed possible patients. In parallel to these studies, the patients were treated and most of them with a Haematopoietic stem cell transplantation (HSCT). During this work, we also evaluated the immune recovery after HSCT repeating some tests performed during the diagnosis. We studied six patients with a suspicion of Severe Combined Immunodeficiency (SCID). After immunological and functional analyses, we studied candidate genes to identify responsible mutations. Two of them presented X-linked SCID (X-SCID) with alterations in the IL-2GR gene: p.Lys120fsX46 and p.Arg222Cys. Patient 3 was deficient in JAK3 with a non described mutation (p.Thr456fsX520). Patient 4 was diagnosed with Reticular Disgenesia after finding a mutation in the AK2 gene (c.A1T). Although patients 5 and 6 presented different clinical manifestations, we established that both siblings had an SCID-Omenn syndrome caused by the mutation c.631delT in the RAG-2 gene. In the analysis of two siblings with suspicion of X-linked Hyper-IgM syndrome, we found a large deletion that prevented CD40L expression. We also studied two patients with X-linked Agammaglobulinemia: patient 9 showed the mutation c.557dupA and patient 10 showed the p.Arg288Gln in the BTK gene. We found a mutation in the WAS gene in the four patients of Wiskott-Aldrich syndrome analyzed: IVS6+5G>A, IVS3+2-3insT, p.Arg34STOP and IVS8+1G>A. The study of the WAS gene allowed also to establish the diagnosis of X-linked Neutropenia in patient 15 (p.Ile294Thr). We studied four patients from two different families with a possible Xlinked Lymphoproliferative syndrome. The three siblings of one of these families presented the mutation p.Val102Gly in the SH2D1A gene, although the youngest had not presented yet symptomatology. In the other patient, we found a complete deletion of the gene. Neutrophil oxidative assay allowed us to diagnose eight patients with Chronic Granulomatous Disease. Afterwards, we analyzed responsible genes until reaching the genetic defect. Patients 20 and 21 presented mutations in the CYBB gene: p.Cys59Arg and p.Lys247fsX7. Patients 22, 23 and 24 were three siblings with the homologue recombination between the NCF1 gene and its pseudogene. Analyses in Patient 25 showed that she was a compound heterozygous of two mutations in the NCF2 gene: p.Lys161STOP and a genetic duplication that included exons 9 and 10. Patients 26 and 27 also showed an alteration in NCF2, but in these cases in homozygosis (p.Arg77STOP). Finally, the study in Patient 28 showed the most prevalent mutation responsible for Incontinentia Pigmenti.
