Abstract

Determinar l'acció d'uns anticossos anti-gangliòsids d'un pacient amb polineuropatia crònica motora desmielinitzant associada a un component monoclonal IgM, sobre conducció nerviosa i sobre la neurotransmissió d'un mamífer adult tant en un estudi agut com crònic i, comprovar que els gangliòsids estan relacionats amb l'entrada de calci al terminal nerviós.<br/>Metodologies emprades:<br/>Determinació de l'especificitat del sèrum del pacient amb tècniques d'ELISA i d'immunoHPTLC utilitzant gangliòsids comercials i gangliòsids purificats de cervell boví, de cervell humà i de cultiu de cèl·lules YAC-1.<br/>Purificació d'anticossos de la classe IgM mitjançant cromatografia d'afinitat amb anticossos de cabra contra IgM humana.<br/>Tècniques d'immunohistoquímica per comprovar la fixació de l'anticòs a la regió presinàptica de la unió neuromuscular.<br/>Inoculació local repetitiva dels anticossos anti-gangliòsid en el múscul LAL de ratolins, cada 24 h durant 15 dies. Utilització de prendisolona per a reduir la resposta inflamatòria local. Els controls es van realitzar amb el mateix protocol però injectant les IgM desnaturalitzades per calor (90 ºC, 20 min) i altres solucions fisiològiques sobre el múscul.<br/>Estudis electrofisiològics convencionals de neurografia motora per determinar potencials motors compostos.<br/>Estudis electrofisiològics intracel·lulars per a comprovar l'efecte del sèrum (i la seva component monoclonal) del malalt tant en incubacions agudes com en els músculs tractats crònicament amb les IgM<br/>anti-GM2.<br/>Resultats i conclusions:<br/>S'ha portat a terme un estudi sobre el paper del sèrum d'un pacient amb una neuropatia crònica desmielinitzant motora pura i un component monoclonal IgM que mostra afinitat per GM2 en la unió neuromuscular. Els estudis electrofisiològics aguts in vitro han mostrat que les IgM del pacient indueixen una reducció de la transmissió neuromuscular que està provocat per un bloqueig en l'entrada de calci a través dels canals tipus P/Q dependents de voltatge, molt probablement alterant algun mecanisme regulador controlat per gangliòsids. En aquest estudi també hem trobat que els gangliòsids contra els que van dirigits els anticossos del malalt es troben habitualment a nivell de sinapsis neuromuscular (tant en l'axó com en la cèl·lula de Schwann) i en el nervi intramuscular (tant en la beina de mielina com en l'axó motor). Pel que fa a l'aplicació crònica dels anticossos anti-gangliòsids del pacient sobre un múscul d'un mamífer adult el que provoquen són alteracions morfològiques (trobem sinapsis amb creixements i/o retraccions axonals) i alteracions funcionals (tenim un bloqueig de la neurotransmissió i l'aparició d'un nou canal de calci) en les unions neuromusculars. Les neurografies motores d'aquest músculs mostren bloqueig de la conducció axonal.


Objective:<br/><br/>To determine the action of anti-gangliosides antibodies of a patient with a chronic motor demyelinating polyneuropathy associated with a IgM monoclonal component on the nerve conduction and on the neurotransmission of a adult mammalian both in a acute and chronic study and, to investigate that the gangliosides are involved in the entry of calcium in the nerve. <br/><br/>Methods:<br/><br/>Determination the serum specificity of the patient with ELISA and immuneHPTLC techniques using commercial and purified gangliosides of bovine brain, human brain and YAC-1 cells. <br/>IgM antibodies purifying with affinity chromatography with goat antibodies against human IgM.<br/>Immunehistochemistry techniques to search the antibody fixation at the presynapticc region of the neuromuscular junction. <br/>Passive transfer of anti-gangliosides antibodies into the LAL muscle of mice, every 24 h during 15 days. Utilization of prendisolone to reduce the inflammatory response. The controls followed the same protocol but the injection contain desnaturalized IgM (90 ºC, 20 min) and other physiological solution on the muscle. <br/>Conventional electrophysiological studies of motor to determinate compost motors action potentials.<br/>Intracellular electrophysiological studies to investigate the serum of patient effect (and the this monoclonal component) both in acutes incubations and in the muscles chronically treated with the anti-gangliosides IgM.<br/><br/>Results and conclusions:<br/><br/>We are made a study about the paper of the serum of a patient with a chronic motor desmielinating polineuropathy associated and an IgM monoclonal component that showed affinity for the GM2 on the neuromuscular junction. The electrophysiological acute studies in vitro have showed that that IgM induced a neuromuscular transmission reduction by the block of calcium entry through the calcium channels dependent voltage P/Q types, probably by the alteration of a regulatory mechanism controlled for gangliosides. In this study we also have found that the gangliosides against the antibodies of patient are normally localized in the region of the neuromuscular synapses (both in the axon and the Schwann cell) and in the intramuscular nerve (both in the myelin and the motor axon). The chronic passive transfer of antibodies anti-gangliosides on the adult mammalian muscle induced morphological alterations, (synapses with sprouts and/or axonal retractions) and functional alterations (neurotransmission block and a new calcium channel were found) on the neuromuscular junctions. In that muscles, the motor neurographies showed axonal conduction block.
