Resumen

El cáncer de mama aumenta con la edad, la esperanza de vida de nuestra población cada vez es más alta , el 50 % de los cáncer diagnosticadas actualmente se presentan en mujeres de > 65 años . El cáncer mama en mujeres mayores presenta una características histológicas e inmunohistoquímicas diferentes de la mujer más joven, presentan con más frecuencia tumores papilares /mucinoso y tubulares, el 85 % de las pacientes presentan tumores que son hormonodependientes, es decir que expresan receptores de estrógenos. Suelen ser tumores a priori más indolentes, Ki 67 bajos (luminal A), Her 2 - . Sin embargo las pacientes mayores presentan una mortalidad por cáncer superior a las mujeres más jóvenes, en principio, se puede explicar con el retraso en el diagnóstico, consultan más tarde. Hasta el 50% de las pacientes mayores se presentan con E II, y la afectación axilar es más frecuente que las mujeres más jóvenes. La edad también se considera un factor de riesgo para recibir tratamientos considerados suboptimos con más frecuencia. Las características inmunohistoquímicas de estos tumores y la falta de obtener un beneficio a largo plazo, hace que estas pacientes reciban con más frecuencia hormonoterapia 1ª , que es una alternativa no curativa al tratamiento quirúrgico . En esta tesis, realizada por compendio de 2 artículos sobre el cáncer de mama en la mujer mayor, analizamos de forma retrospectiva, las pacientes de ≥ 80 a., tratadas en el centro de cáncer de mama de Vall d Hebrón, comparamos las mujeres que han recibido hormonoterapia 1ª vs las pacientes que fueron intervenidas quirúrgicamente (y recibieron hormonoterapia adyuvante). También comparamos si el tipo de cirugía recibida influye en el pronóstico de estas pacientes, ya que las pacientes mayores reciben muchas veces tratamientos quirúrgicos considerados suboptimos. Se observo que ambos grupos (hormonoterapia 1ª y cirugía) eran homogéneos para: otros tratamientos recibidos (quimio, radioterapia), Her 2, grado histológico, que lo eran (excepto grado histológico que era algo mayor en el grupo quirúrgico). La comorbilidad de las pacientes podía actuar como una sesgo a la hora de comparar los grupos (las más frágiles recibían hormonoterapia), para ello se estudió la comorbilidad de las pacientes mediante una escala simple y objetiva como la escala de Charlson, ambos grupos (hormonoterapia 1ª y cirugía) eran homogéneos en cuanto a ala comorbilidad. Las pacientes se estratificaron en estadios iniciales y estadios localmente avanzados. En nuestra serie, las pacientes en estadios iniciales que fueron sometidas a tratamiento quirúrgico presentaron una supervivencia específica por cáncer de mama (SBCS) de 108 meses (95% CI 101-115), el grupo que recibió tratamiento sistémico (39) presentó una SBCS de 50 meses (95 % CI 39-61), siendo las diferencias estadísticamente significativas. Las pacientes en estadios localmente avanzados no presentaron diferencias significativas entre el grupo quirúrgico y hormonoterápico. Dentro del grupo de cirugía se observo que las pacientes que recibían un tratamiento quirúrgico óptimo o estándar presentaban un tiempo libre de enfermedad y una SBCS, superior a las pacientes que no recibían un tratamiento quirúrgico óptimo. La valoración la paciente mayor debe empezar realizando un asesoramiento geriátrico, que valore la esperanza de vida y la comorbilidad de la paciente. Aquellas pacientes con alta comorbilidad y esperanza de vida < 2-3 años la hormonoterapia es una buena opción. Las pacientes con buena esperanza de vida y comorbilidad aceptable deben recibir un tratamiento oncológico y quirúrgico estándar y aceptable.


The risk of breast cancer increases with age, and the increased life expectancy in today’s society has shown a rise in breast cancer cases for women older than 80 years. 30% of breast cancer occurs in women over 75 years Age is an independent risk factor determining a non standard treatment. The breast cancer in elderly patients is different from younger patients, the hormone status is positive in 85 % of patients, the over expression of Her 2 is less frequent (10%). Histologically, these patients have more indolent patterns as tubular, mucinous and papillary tumors. In contrast, 50% of elderly patients are in II stage, and axillary involvement is also more frequent than younger patients (partly explained for a delayed diagnosis). The specific breast cancer survival in elderly patients is lower than younger patients. Elderly patients receive more often, primary endocrine treatment (PET) alone, PET without surgery is a non-curative alternative treatment to primary surgery + hormonal therapy (PST) The aim of these two studies is to compare the breast-cancer specific survival between PET and PST. Both groups (PET and PST) were homogenous regarding to the expression of ER, Her2 over expression, limphovascular invasion and the subsequent adjuvant therapy (chemotherapy and radiotherapy). The tumoral grade was greater in the PST group. The comorbidity in elderly patients , is very important , because it could act like bias to decide the treatment, we studied de comorbidity using de Charlson score because is simple and objective , the Charlson comorbidity scale was 5 for the PST group and 4.6 for the PET group, which did not show significant differences We stratified both groups (PST and PET) in early stages (E II) and locally advanced stages (E III) and we studied the BCSS in both groups (PST and PET). In the PST group we also studied if the type of surgery, (standard or suboptimal) impacts on survival. From patients in early stages, PST group has a greater BCSS than the PET group, ant the differences were statistically significant. From patients in locally advanced stages, PST group has also a greater BCSS than PET group, differences were not statistically significant. In the PST group, according to the type of surgery performed, we found that the disease free survival and the BCSS were greater in the group that received an optimal surgery than the group considered sub-optimal. The evaluation of elderly patient should begin with a geriatric assessment, to assess the life expectancy and comorbidity of these patients. if the life expectancy of the patient is <2-3 years or are considered unfit for anesthesia, the optimal treatment is primary hormonal therapy, avoiding the morbidity of surgery. Patients with a good or reasonable life expectancy, considered fit to anesthesia , the should receive a standard oncologic / surgery treatment as younger patients.
