Resumen

La Miastenia Gravis (MG) es una enfermedad autoinmune mediada por anticuerpos frente a antígenos de la unión neuromuscular. El 80% de los pacientes presentan anticuerpos antireceptor de acetilcolina, un 5% presenta anticuerpos anti-tirosin-kinasa específica de músculo (MuSK) y un 15% no presenta anticuerpos frente a estos dos antígenos, entidad conocida cono MG seronegativa. En el caso de las formas oculares puras, hasta un 50% de los pacientes son seronegativos. El diagnóstico inmunológico de los pacientes tiene implicaciones diagnósticas, pronósticas y terapéuticas. En el caso de la MG anti-MuSK, la descripción de los anticuerpos definió un subgrupo de pacientes con unas características clínicas comunes y una excelente respuesta a rituximab. En el primer artículo de esta tesis se describe la frecuencia de los anticuerpos anti-cortactin en MG seronegativa así como el fenotipo clínico de estos pacientes. En nuestra serie, los anticuerpos anti-cortactin están presentes en un 24% de pacientes con MG seronegativa, un 9.5% de los pacientes con MG anti-RACh (porcentaje significativamente menor), y no se asocian con anti-cuerpos anti-MuSK, anti-LRP4 o anti-musculo estriado. Este hecho sugiere que los anticuerpos anti-cortactin no son consecuencia de epitope spreading. Los pacientes con MG seronegativa y anticuerpos anti-cortactin tienen un fenotipo ocular o generalizado leve, MGFA I o IIA, sin síntomas bulbares. Por tanto, la presencia de los anticuerpos anti-cortactin ayuda en el diagnóstico de la MG autoinmune, especialmente en los casos de MG ocular, y apoya el tratamiento con inmunosupresores si es clínicamente necesario. El segundo artículo de esta tesis demuestra que la posología de rituximab utilizada en el tratamiento de la MG anti-MuSK fármacorresistente determina la frecuencia de recaídas y la duración del efecto beneficioso. Se conoce que el rituximab es una opción terapéutica eficaz para pacientes con MG anti-MuSK resistentes a otros fármacos inmunosupresores, sin embargo la posología utilizada para el tratamiento de los pacientes con MG anti-MuSK varía mucho entre diferentes centros. En este estudio, se compara el efecto beneficioso de tres posologías de rituximab diferentes. Todos los pacientes lograron un MGFA-PIS de remisión completa estable, remisión farmacológica o manifestaciones mínimas, y la prednisona y otras terapias inmunosupresoras se retiraron o redujeron en todos los casos. Sin embargo, el número de recidivas y la duración de la respuesta fueron significativamente diferentes dependiendo del protocolo de rituximab utilizado. El protocolo 4+2 (375 mg/m2 cada semana durante 4 semanas consecutivas y luego mensualmente durante los siguientes 2 meses) de rituximab demostró tener la tasa de recidiva clínica más baja y el efecto más duradero comparado con los protocolos 1+1 (dos dosis de 1 g separadas por 2 semanas) y 4 (375 mg/m2 cada semana durante 4 semanas consecutivas). Por este motivo, en MG con anticuerpos anti- MuSK proponemos estandarizar el tratamiento con rituximab utilizando el protocolo 4+2. En resumen, esta tesis aporta conocimiento sobre nuevos biomarcadores diagnósticos y pronósticos en MG. Los hallazgos de esta investigación son directamente aplicables a la práctica clínica y abren interesantes nuevas líneas de investigación en el campo.


Myasthenia gravis (MG) is an autoimmune disease mediated by antibodies against antigens of the neuromuscular junction. Around an 80% of patients have anti-acetylcholine receptor antibodies (RACh), 5% have anti-muscle-specific tyrosine kinase (MuSK) antibodies and 15% have no antibodies to these two antigens, an entity known as seronegative MG. In pure ocular forms, up to 50% of patients are seronegative. The immunological diagnosis of patients has diagnostic, prognostic and therapeutic implications. In the case of anti-MuSK MG, the finding of the antibodies defined a subgroup of patients with common clinical characteristics and an excellent response to rituximab. The first article of this thesis describes the frequency of anti-cortactin antibodies in seronegative MG as well as the clinical phenotype of these patients. In our series, anticortactin antibodies are present in 24% of patients with seronegative MG, 9.5% of patients with anti-RACh MG (a significantly lower percentage), and do not associate with anti-MuSK, anti-LRP4 or anti-striated muscle antibodies. This fact suggests that anti-cortactin antibodies are not a consequence of epitope spreading. Patients with seronegative MG and anti-cortactin antibodies have a mild ocular or generalized phenotype, MGFA I or IIA, without bulbar symptoms. Therefore, the presence of anti-cortactin antibodies helps in the diagnosis of autoimmune MG, especially in cases of ocular MG, and supports treatment with immunosuppressants if clinically necessary. The second article of this thesis shows that the posology of rituximab used in the treatment of drug-resistant anti-MuSK MG determines the frequency of relapses and the duration of the beneficial effect. It is known that rituximab is an effective therapeutic option for patients with anti-MuSK MG resistant to other immunosuppressive drugs; however the posology used for the treatment of patients with anti-MuSK MG varies between different centers. In this study, the beneficial effect of three different rituximab posologies is compared. All patients achieved a stable complete remission, pharmacological remission or minimal manifestations MGFA-PIS, and prednisone and other immunosuppressive therapies were withdrawn or reduced in all cases. However, the number of relapses and the duration of the response were significantly different depending on the protocol of rituximab used. The 4+2 protocol (375 mg/m2 every week for 4 consecutive weeks and then monthly for the next 2 months) of rituximab proved to have the lowest clinical recurrence rate and the most lasting effect compared to the 1+1 protocol (two doses of 1 g separated by 2 weeks) and 4 protocol (375 mg/m2 each week for 4 consecutive weeks). For this reason, in MG with anti-MuSK antibodies we propose to standardize treatment with rituximab using the 4+2 protocol. In summary, this thesis provides knowledge on new diagnostic biomarkers and prognosis in MG. The findings of this research are directly applicable to clinical practice and open up interesting new lines of research in the field.
